What is Sistema Tessera Sanitaria?

Sistema TS (Tessera Sanitaria) is Italy's tax authority portal where healthcare professionals must report patient medical expenses. This data is used for pre-filled tax returns.

Deadlines and Penalties 2026

When to submit? (2026 Update)

From 2026, submission is annual: by January 31 of the following year (for 2025 expenses: deadline February 2, 2026). You have 5 days after the deadline to correct errors without penalties.

Penalties for non-submission

€100 per document omitted or incorrect (max €50,000). With early correction within 60 days: only €3.70 per document.

Warning: Legal accumulation doesn't apply — each document generates a separate penalty. 50 forgotten invoices = €5,000 fine.

How long does setup take?

Is patient data secure?

Yes. Servers in the EU, encrypted connections and field-level AES-256 encryption of specific data, such as patients' tax codes in your client records, the service descriptions sent to Sistema TS and your Sistema TS credentials. Processing complies with the GDPR and healthcare data is never disclosed publicly.

What if I already have an accountant?

FatturaSmart doesn't replace your accountant. It only automates submission to Sistema TS, freeing you from this tedious task and saving you valuable time.

How does automatic Sistema TS submission work?

When you create an invoice, FatturaSmart automatically sends it to Sistema Tessera Sanitaria. If there are errors, it automatically retries until successful. You don't have to do anything.

How does the 30-day free trial work?

For 30 days you use every feature of the plan with no charge. If you're not satisfied, cancel from the app within the 30 days and you pay nothing; if you stay, the annual subscription starts when the trial ends.
